In most cases, a total treatment fee is presented to you at the end of your consultation visit. The cost of treatment may be paid in monthly installments over the approximate treatment time period. The method of payment will be discussed with you at the start of treatment.

Insurance

At the time of your first visit to Dr. Bilodeau’s office, please bring any insurance forms related to your treatment. Our office does accept direct assignment from the insurance company, and we will be glad to process and submit insurance claims for you at no additional charge. Orthodontic treatment claims generally fall under dental insurance, while TMJ or surgical claims may fall under medical insurance. Please bring your insurance form after completing this checklist:

1. Patient information is filled out completely (name, address, Social Security number, etc.).
2. Please sign the form where indicated (most forms require two signatures).

Insurance coverage for orthodontics can vary greatly between plans. Most cover a part of treatment up to a lifetime maximum amount.

Our business office will be most happy to process a pre-treatment authorization for you to determine what part of the financial aspects of treatment will be reimbursed by your insurance company. Many insurance companies require pre-authorization. Therefore, please check with your insurance company for verification of benefits.
